If you are a new patient or have not been seen in the last 3 years, your insurance is an HMO, or if you are being seen under VA, Workers’ Compensation, or Auto insurance, you are not eligible to usePatient Self‑Scheduling. Please call our office at 941‑485‑1505 to schedule an appointment.
If you do schedule with any of these circumstances, your appt will be cancelled and subject to our $50 cancellation fee.
