If your insurance is an HMO, or if you are being seen under VA, Workers’ Compensation, or Auto insurance, you are not eligible to use Patient Self‑Scheduling. Please call our office at 941‑485‑1505 to schedule an appointment.

If you do schedule with any of these insurances, your appt will be cancelled and subject to our $50 cancellation fee.
